Plan includes:

• Two professional prophies (cleanings)
• Two professional exams
• One set of bitewing radiographs
• All emergency exams
• Plus, two fluoride treatments for children

Important note: This plan expires one year from the date of purchase. It is also nontransferable and nonrefundable.

Price Breakdown:

Adults: $295.00 per year
Child: $220.00 a year per child (17 years or younger)

If you receive a periodontal prophy here at our dental office, you will receive the fee of regular prophy off your first and second prophy of the year. Please note, you will have an additional fee of $65.00 at your first and second visit with us. On your third or fourth visit, you will receive the appropriate savings from your form of payment.

For all other services we provide, if you pay in full with cash or check, you can receive 25% savings and 20% savings with a credit card.

No savings will be applied for CareCredit® patients.
